Form 93 is mandatory for all Indian individual citizens applying for a fresh PAN card starting April 1, 2026. This includes:
- Salaried employees applying for PAN for the first time
- Self-employed professionals and freelancers
- Individuals converting from a Minor PAN to an adult PAN (Minor to Major)
- Any Indian citizen who does not yet have a PAN card
How to Fill Form 93 — Step-by-Step
- Full Legal Name: Enter your full legal name exactly as it appears on your Aadhaar card. Use BLOCK LETTERS only. Do not abbreviate.
- Father's / Mother's Name: New options for single parents are now clearly defined. Fill the appropriate field based on your situation.
- Date of Birth: Enter in DD/MM/YYYY format. Do not use slashes or dashes — write digits only in the boxes provided.
- Gender & Status: Select your gender and confirm your Indian residency status accurately.
- Address Details: Use your current residential address. This must match Aadhaar or your proof of address document.
- Photograph Placement: Affix two 3.5cm × 2.5cm colour photographs on the designated boxes. Do not staple — use glue.
- Signature / Thumb Impression: Sign across the photograph in the specified box. Ensure the signature stays within the lines.
- Supporting Documents: Attach all required proof of identity and address (Aadhaar + one secondary document) as per the updated 2026 rules.
Mandatory Documents After April 2026
Following the 2026 update, Aadhaar alone is no longer sufficient. You must provide at least two documents:
- Proof of Identity (POI): Aadhaar Card (mandatory) + Voter ID / Passport / Driving Licence
- Proof of Birth (DOB): Birth Certificate / Matriculation Certificate / Passport / Aadhaar
- Proof of Address (POA): If your Aadhaar address is outdated — Utility Bill / Bank Statement

- Name mismatch between application and Aadhaar — even a single letter causes rejection
- Using pencil or coloured ink to fill the form — always use black or dark blue ink
