What is Form 94 and Who Needs It?

Starting April 1, 2026, Form 94 replaces the old correction form for all changes and corrections in PAN data for Indian individuals, companies, firms, trusts, and other entities. If there's any error on your existing PAN card — name spelling, date of birth, address, or signature — you must use Form 94 to get it corrected.

What Can Be Corrected with This Form?

• Spelling mistakes in your name or parents' names
• Incorrect Date of Birth (DOB)
• Gender or category errors
• Address update (change of city or state)
• Photo update (for Minor-to-Major conversion)
• Signature addition or update

Step-by-Step Correction Process

1. Download the form using the link above. Print on A4 size paper — do not scale or resize.
2. Tick the field(s) at the top of the form that you wish to correct — only tick what needs changing.
3. Fill the corrected data in Block Letters using black ink in the corresponding fields.
4. Attach proof documents that reflect the corrected information. For example, if correcting DOB — attach a certified Birth Certificate or Matriculation mark sheet.
5. Affix your self-attested photograph and sign across it, if required by the correction type.
6. Submit or apply online through our correction portal for faster processing.

Important Rules for 2026

The most important new rule: the name and DOB you enter in the correction form must exactly match the supporting documents you attach. Even a minor discrepancy (e.g., "Md." vs "Mohammad") will cause immediate rejection.
